IRIS Self-Directed Personal Care (RN) (Racine County, WI & Kenosha County, WI)

Location: Racine, WI

Schedule: Full Time, Day shift

What You'll Do: Provide personal care assessments and oversight using the Wisconsin Personal Care Screening Tool. Document assessments as required by individual tools and Department of Health Services policies. Oversee a group of participants, developing individual plans of care and ensuring physician orders are obtained. Submit requests for Prior Authorization for personal care services. Comply with all Department of Health Services policies and SDPC guidelines. Provide personal care training to participants or care providers as requested. Complete collateral contacts with IRIS Consultants and Long-Term Care Functional Screeners. Complete other duties as assigned. Drive 50% of the time for member home visits. Navigate various environments and potential obstacles in members' homes. RequirementsEducation: Associate's degree in nursing required.

Experience: At least 2 years nursing experience, with 1 year serving target groups of the IRIS program.

Licenses & Certifications: Active and unrestricted Registered Nurse (RN) license in Wisconsin. Valid driver's license and reliable transportation.
