Home Health Physical Therapist

Mercy is seeking a full-time Physical Therapist to provide one-on-one home health care to patients in Festus. In this role, you will evaluate functional abilities, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility, strength, and quality of life. You will coordinate with nurses and physicians, educate patients and caregivers, and document care in EMR. Provide in-home physical therapy evaluations and treatments for adult and geriatric patients in Festus Develop and update individualized, evidence-based care plans to improve function and mobility Document assessments, goals, and progress in EMR according to home health regulations Educate patients and caregivers on exercises, safety, and use of assistive devices Collaborate with physicians, nurses, and other clinicians to coordinate comprehensive care Monitor patient progress, modify interventions, and escalate concerns as needed Ensure compliance with clinical, safety, and infection control standards Participate in professional development and quality improvement initiatives Patient assessment Therapeutic exercise planning Manual therapy techniques Home health documentation (EMR) Care plan development Gait and balance training Pain management modalities Patient and family education Interdisciplinary collaboration Safety and fall-risk assessment Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active state license as a Physical Therapist (PT) in MO Graduation from an accredited Physical Therapy program (DPT or equivalent) Current CPR/BLS certification Valid driver’s license and reliable transportation for home visits OASIS/home health documentation experience preferred
