Home Infusion Clinical Pharmacist

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe, effective pharmacy services for home care patients. You will coordinate with Nursing and other teams, monitor patient progress, and educate patients, caregivers and staff on medication use. The role emphasizes pharmacokinetic assessment, lab value interpretation, and adherence to regulatory standards in an alternate-site setting. A pharmacy license and PharmD are required.
