Home Health Registered Nurse

Piedmont Healthcare is seeking a compassionate Registered Nurse to join our Augusta Home Health team in Martinez, GA. In this role, you will provide skilled, patient-centered nursing care in the home setting, including assessments, care planning, wound care, medication management, and patient/family education. You will collaborate closely with an interdisciplinary team to ensure safe transitions of care and optimal outcomes. Perform comprehensive nursing assessments in patients’ homes and update care plans based on clinical findings and physician orders. Provide skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management. Educate patients and caregivers on medications, treatment plans, equipment use, and symptom monitoring to promote self-management and prevent complications. Coordinate care with physicians, therapists, social workers, and other team members to ensure safe, effective, and timely services. Monitor patient progress, recognize changes in condition, and intervene or escalate care appropriately to prevent hospital readmissions. Document all assessments, interventions, and patient education accurately and promptly in the electronic health record (EHR). Manage a flexible caseload and daily visit schedule, prioritizing patient needs while adhering to regulatory and agency requirements. Ensure compliance with all home health, safety, infection control, and quality standards as set forth by Piedmont Healthcare and regulatory bodies. Communicate regularly with patients, families, and the care team to support patient-centered goals and address concerns. Represent Piedmont Healthcare professionally in the community, supporting a culture of respect, collaboration, and continuous improvement.
