Nurse Practitioner Opportunity in Warwick, RIHeartline Staffing is hiring a Nurse Practitioner in Warwick, Rhode Island. Provide equitable, effective value-based healthcare to local Medicare patient populations. Supported by a large care team so you can focus on quality of care rather than volume of services. Value-based model and bonuses structured to reward outcomes, low hospital admissions, preventive medicine, and an outstanding patient experience. Collaborative clinics with an average of 35 providers, a mix of Nurse Practitioners and Primary Care Physicians. Base salary of $120,000$140,000. Incentive bonus of up to $8,000. Monday through Friday, 8:00 AM5:00 PM. 1316 patients per day with 20-minute regular visits and 40-minute new patient and full diagnostic visits. Dedicated care team including a scribe, medical assistant, and social worker. 6 weeks PTO, inclusive of PTO, major holidays, and CME. Monday through Friday. 8:00 AM5:00 PM. 1316 patients per day. 20-minute regular visits; 40-minute full diagnostic and new patient visits. Patients seen on average every 3 months, with more frequent visits for complex patients. Care team: scribe, medical assistant, and social worker. On-site labs with a phlebotomist and access to specialist consultations. Van transportation for patients within 5 miles of the clinic. Community room activities for patients, including exercise, events, yoga, dance, and spa days. Value-based care model. Provide value-based primary care to Medicare patients. Deliver preventive medicine and work to reduce hospital admissions. Manage ongoing care for patients, including frequent follow-up for complex patients. Collaborate with physicians, fellow Nurse Practitioners, and the care team. Document accurately to support quality, coding, and risk assessment.
