Certified Home Health Aide, Hospice (per diem)

Per Diem Certified Home Health Aide (CHHA) Assisted Hospice is looking for a per diem Certified Home Health Aide (CHHA) for our Los Angeles/Culver City branch. We have about 10-15 visits per week available. We just increased our visit rates for CHHA to $29 per visit + $11 for mileage. The Impact You Will Make: Assist patient with personal hygiene, elimination, nutritional support, ambulation and exercise according to the patient's needs and as specified on the CHHA Assignment. Ensure quality and safe delivery of CHHA services per written plan of care. Communicate with RN, Rehab Case Manager or Clinical Supervisor on a regular basis regarding changes in the patient's condition and/or needs.
