Outpatient Physical Therapist:

  • Schedule5x8.0 shifts; flexible schedule, with 1-2 evenings until 7 PM.
Job Description: The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient’s needs, goals, and physician direction. Responsibilities include the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that allow effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves.
Responsibilities: Complete comprehensive evaluations of assigned patients. Utilize clinical reasoning to develop appropriate treatment plans and goals. Document medical records in a clear, concise, accurate, and timely manner. Ensure care is provided under signed, valid physician orders and within insurance approvals. Participate in student program as assigned, providing a positive learning environment. Create an individualized professional development plan with clinical management. Adhere to discipline-specific State Practice Act and other regulatory agencies. Understand and follow the organization’s performance improvement methodology.
