Physical Therapist Assistant

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team supporting seniors in skilled nursing and post-acute settings. Under PT supervision, you’ll deliver individualized treatments to improve mobility, strength, and function, while documenting outcomes and collaborating with an interdisciplinary team. Provide treatment to senior patients under the supervision of a Physical Therapist Implement individualized care plans to improve mobility, strength, and function Document treatments, progress notes, and outcomes accurately in EMR systems Collaborate with PTs, nursing, and interdisciplinary teams on patient care Educate patients and families on exercises, safety, and functional goals Support a patient-centered, compassionate approach consistent with Renewal Rehab’s values Therapeutic exercise implementation Gait and balance training Modalities application (e.g., heat, cold, e-stim) Documentation in EMR systems Functional mobility and transfer training Patient and family education Care planning with supervising PT Knowledge of SNF/post-acute regulations Infection control and safety practices Outcome measurement and reporting Medical Insurance Dental Insurance Vision Insurance 401k Active state PTA license or eligibility Graduation from an accredited PTA program Current BLS/CPR certification Familiarity with SNF compliance and documentation standards
