Full-Time | Non-ExemptAbout the RoleWe are seeking a compassionate and experienced registered dietitian (RD/RDN) to join our PACE (Program of All-Inclusive Care for the Elderly) team in San Francisco. As a key member of our interdisciplinary care team, you will provide nutrition assessments, counseling, and individualized care plans that help older adults maintain their health, independence, and quality of life while aging safely in their communities. This position is ideal for a dietitian who is passionate about working with seniors, values a collaborative team environment, and is committed to delivering culturally sensitive, person-centered care.
Essential Responsibilities:
Perform comprehensive nutrition assessments and provide medical nutrition therapy (MNT) for PACE participants.
Develop, implement, and monitor individualized nutrition care plans and therapeutic diets.
Provide nutrition education and counseling to participants, families, and caregivers.
Create meal plans that align with participants' medical conditions, cultural preferences, and lifestyle needs.
Document assessments, interventions, and follow-up care in Epic and other electronic health record systems.
Collaborate closely with physicians, nurses, social workers, rehabilitation staff, and other interdisciplinary team members.
Conduct home visits and community-based assessments as needed.
Coordinate nutrition-related referrals and community resources to support continuity of care.
Develop educational materials, clinical protocols, and nutrition-related resources.
Participate in quality improvement initiatives and program development activities.
Monitor participant progress and adjust care plans to support optimal health outcomes.
Qualifications:
RequiredBachelor's degree in nutrition, dietetics, or a related field.
Current Registered Dietitian (RD/RDN) credential in good standing.
Current BLS and first aid certification.
Experience with electronic health records (EHR) and strong computer proficiency.
Excellent verbal and written communication skills in English.
Strong assessment, counseling, and care coordination skills.
Preferred:
Master's degree in nutrition, dietetics, or a related field.
Experience working with geriatric, frail, or medically complex populations.
At least one year of experience serving older adults.
Two or more years of experience in nutrition assessment, counseling, and patient education.
