Hybrid Veteran Care Navigator & VA Liaison

Sanford Health in the heartland seeks a Veteran Service Care Coordinator to bridge the VA, Marshfield Clinic Health System, and community, ensuring Veterans access to services, benefits, and smooth transitions of care. You will screen for medical and social needs, develop action plans around VA eligibility and preventive health goals, and coordinate with clinicians, discharge planners, and community partners to support Veteran care.
