Position Overview: We are seeking a physician (MD/DO) to provide on-site primary care to skilled nursing facility residents while serving as Medical Director for assigned facilities. Physicians are supported through Telos' fully funded Certified Medical Director (CMD) training and a strong clinical infrastructure. Clinical Responsibilities: Provide on-site primary care to skilled nursing facility residents Evaluate residents for significant changes in condition Collaborate with interdisciplinary care teams and facility leadership Supervise and support NP/PA clinicians Participate in a defined, structured after-hours call schedule Provide or oversee after-hours telehealth care

Medical Director Responsibilities: Serve as Medical Director for assigned skilled nursing facilities Participate in fully funded Certified Medical Director (CMD) training Support Quality Assurance & Performance Improvement (QAPI)Promote regulatory compliance and survey readiness Provide interdisciplinary clinical leadershipCMD certification is not required to start. All CMD training expenses are fully funded.
