Job Description: Seeking a dedicated Nurse Practitioner with home health experience to conduct risk assessments for Medicare Advantage plan members across Jackson County. This per diem opportunity is ideal for those looking for a flexible role with autonomy and a meaningful impact on patient care.

Key Responsibilities:
  • Perform annual home health risk assessments across various locations in Jackson CountyAssess previous and current medical history, conditions, and medication use
  • Provide valuable education regarding health conditions and prescriptions to patients
  • Identify any care gaps and offer actionable recommendations for follow-upVisits typically last 45 minutes and may include annual assessments or post-discharge evaluations
  • No medication prescribing or treatment planning involved
Qualifications:
  • Active Nurse Practitioner License in Ohio
  • Current BLS certification (CPR)
  • Board Certification through AANP or ANCCMinimum of 6 months of recent experience as a Nurse Practitioner preferred
  • Masters degree in Nursing (MSN) from an accredited institution
Compensation for this position ranges from $80 to $90 per completed evaluation, along with a mileage stipend and payment for administrative time.
