Health Home Care Coordinator (Case Management)

Rochester Regional Health's St. Mary’s Campus is seeking a full-time Health Home Care Manager to provide care coordination for high-need enrollees, addressing medical, behavioral, social and educational needs. This role involves creating and managing comprehensive care plans and ensuring documentation within deadlines. The position covers a mid-acuity caseload including AOT clients, requires a BS in Social Work, strong analytical and communication skills, and a NYS driver license.
