In-Home Health NP/PA Flexible Travel & Benefits

Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to perform In-Home Health Evaluations. The role involves visiting members at their homes, reviewing medical history, conducting physical exams, and coordinating with health plans. You will use an iPad and a clinical workflow, with some virtual visits as needed. Required are active unrestricted licenses, board certification, and multi-state licenses preferred.
