Prenatal Services & Education Coordinator

Via Care Community Health Center is seeking a CPSP Coordinator in Los Angeles to assist in prenatal case management and program operations. You will conduct assessments across prenatal stages, support nutrition and health education, and coordinate referrals and follow-up care. The role requires a High School diploma with CPHWCPHW Certification, and preferred bilingual abilities for diverse communities. This is a full-time, weekday position with standard office hours and on-site work in LA.
