Compassionate In-Home Hospice Chaplain

Advocate Health at Home seeks a Chaplain to provide holistic spiritual care to patients, families, and staff in home-based hospice. The role includes counseling, crisis response, sacramental support, and collaboration with interdisciplinary teams to ensure compassionate, comprehensive care. The chaplain will travel within the Advocate Aurora service area, participate in on‐call rotation, and support patients and families from diverse backgrounds during end‐of‐life journeys.
