Job Title Physical Therapist Location Colorado | Outpatient Schedule / Job Type Contract | Days | 6-Month Assignment Key Responsibilities Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction Manage a caseload of up to 2 patients per hour, with initial evaluations and safety patients receiving a full hour of one-on-one care; standard sessions are generally 45 minutes or more Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, providing appropriate supervision and completing student evaluations Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements Required Qualifications Active Physical Therapist (PT) license in good standing in the state of Colorado Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans Proficiency in clinical documentation, goal writing, and compliance with state, federal, and payer guidelines Commitment to patient and employee safety standards and adherence to professional codes of conduct Preferred Experience / Skills Prior experience in an outpatient physical therapy setting Experience supervising or mentoring PT students Demonstrated involvement in continuing education and professional development activities Why Join Us Opportunity to work in a structured outpatient environment with a focus on delivering comprehensive, individualized patient care Supportive clinical setting that values professional growth, patient safety, and team collaboration Concentric Healthcare Staffing Job ID.
