In-Home Geriatric Social Worker - Flexible Schedule

21st Century Home Health Services is seeking a Medical Social Worker (MSW) to visit patients in their homes across Sonoma County and surrounding areas. The MSW will assess social and emotional needs, educate families, and coordinate with the care team to integrate services for optimal patient outcomes. The ideal candidate has an MSW, is prepared to pursue LCSW, and can navigate local resources to support geriatric patients and their caregivers.
