Adult Nurse Practitioner- Dyersburg, TN

Key Responsibilities: Conduct in-home patient visits to assess clinical status and provide post-discharge medical care. Review and interpret discharge summaries and progress notes. Collaborate closely with home health professionals and other members of the care team to ensure seamless coordination of care. Provide medical consultations, prescribe medications when appropriate, and oversee ongoing care plans to promote recovery and reduce readmissions.

Qualifications: Current Tennessee medical license and active Medicare provider number required. Nurse Practitioner certification in good standing. Ability to travel within a 35-mile radius from home to see patients throughout Dyersburg, Dyer County, and surrounding areas. Strong communication, clinical judgment, and documentation skills. A compassionate, faith-driven approach to patient care.

Base Salary: $115,000 – $135,000 (commensurate with experience)
