Schedule: Weekday 8am 5pm, some night call, weekend call Practice Setting: Outpatient Types of Cases: Standard vascular surgery cases Credentialing Timeframe: 60 days Electronic Medical Record (EMR): Epic Certifications Required: Must be board certified or truly board eligible Licensure Required: Advanced Cardiovascular Life Support (ACLS), Basic Life Support (BLS), Drug Enforcement Administration (DEA), Active Illinois state license Facility Location Known as The Forest City, Rockford is located just outside of the Chicago metro area.
