Responsibilities: Design and implement personalized nutrition plans for patients with various medical conditions, including diabetes, heart disease, and obesity Conduct nutrition assessments and counseling sessions with patients and their families Collaborate with healthcare teams to develop and implement evidence-based nutrition therapy plans Develop and maintain accurate and complete patient records, including nutrition plans, assessments, and progress notes Stay current with the latest research and guidelines in nutrition and dietetics to ensure optimal patient care Participate in quality improvement initiatives to optimize patient outcomes and hospital processes
