General Description:
  • Review and evaluate clinical assessments and UAS information for MLTC/MAP members.
  • Develop, implement, and monitor individualized member care plans.
  • Coordinate care with PCPs, physicians, nurses, hospitals, and other healthcare providers.
  • Identify clinical risks and ensure appropriate interventions to prevent unnecessary hospitalizations and ED visits.
  • Monitor member health status and coordinate preventive and chronic disease management.
  • Specific Skills to be Added(In case of CB)
  • Active New York State RN License.
  • Minimum 5 years of clinical experience as a Registered Nurse.
  • Hospital experience.
  • Care coordination / case management experience.
  • Bilingual in English and Spanish (Spanish-speaking required).
