Autonomous Home Palliative Care Nurse Practitioner

Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to provide advanced home-based palliative care, collaborating with a Palliative Medical Director to optimize quality of life for patients with complex needs. You will perform comprehensive assessments, develop individualized care plans, prescribe medications per state law, document accurately, and participate in after-hours call rotations while supporting families through treatment decisions.
