California Licensed Health Navigator

Key responsibilities: Conduct member outreach, assess needs, and support care coordination across the care continuum Educate members on benefits, eligibility, referrals, and service utilization Coordinate with providers, caregivers, and internal teams to ensure continuity of care

Required qualifications: 3+ years of experience in healthcare and/or healthcare customer service (managed care/health plan preferred) 1+ year of experience supporting care coordination or discharge planning preferred Strong knowledge of medical/insurance terminology and member-focused communication style Proficient with Microsoft Office (Word/Excel/Outlook) and web-based systems TB clearance required prior to or within seven (7) days of hire (as applicable)
