Empowering Eligibility Specialist & Care Manager

Care Lync is seeking an Eligibility Specialist / Case Manager to provide case management and service coordination for adults with disabilities enrolled in the Medicaid Waiver Program. You will develop individualized care plans, broker services, and monitor outcomes to ensure cost-effective, professional care. The role includes coordinating housing, medical, social, vocational, and other supports, with approximately 50% travel to participants as needed.
