Key responsibilities:
Utilize clinical expertise to provide health support and solutions for members with chronic health conditions
Perform telephonic assessments to evaluate member needs and readiness to change
Engage members in developing wellness goals and monitor their progress to ensure effective care plans
Required qualifications:
Associate's degree or higher; Master's degree in Mental Health, Counseling, Social Work, or Nursing preferred
2-4 years of related behavioral health or psychiatric experience
Current Texas state licensure as an LPC, LCSW, LMSW, LMFT, or RN required
Experience in psychiatric intensive outpatient programs or managed care behavioral health management
Proficient in Microsoft Office applications; bilingual in English and Spanish is a plus
