Home Health and Hospice Licensed Vocational Nurse (LVN)

VitalCaring is seeking a Licensed Vocational Nurse to deliver compassionate, clinically excellent care to home health and hospice patients. In this field-based role, you’ll provide skilled nursing services including assessments, medication administration, wound care, and patient education under RN and physician direction. You will coordinate with an interdisciplinary team, document care accurately, and support patients and families in managing chronic conditions and end-of-life needs. Provide skilled nursing care to home health and hospice patients under physician orders and RN supervision. Monitor and document vital signs, symptoms, and patient responses in EMR systems. Administer medications and treatments, including wound care and injections, per care plan. Educate patients and families on disease management, medications, and home care techniques. Collaborate with RNs, therapists, social workers, and physicians to update individualized care plans. Perform home visits, assess safety and needs, and escalate changes in condition promptly. Maintain accurate, timely clinical documentation to meet regulatory and agency standards. Follow infection control, safety, and quality protocols in all patient interactions.
