Transitional Care Coordinator

Responsibilities: Coordinate Cottage Recovery, Private Duty Home Care, and Wellness Clinic services Coordinate admissions, care transitions, and resident services Collaborate with physicians, healthcare providers, residents, and families Educate residents and families on care options, medications, insurance, and available resources Assist with care coordination, documentation, and regulatory compliance Connect residents with community and social service resources
