Inpatient RN Case Manager — Discharge & Care Coordination

Stanford Health Care in Palo Alto, CA seeks a Clinical Case Manager (RN) to coordinate care and discharge planning for a caseload across inpatient units. You will lead utilization review, review charts for timeliness and appropriate services, and collaborate with physicians, social workers and the care team to ensure efficient, high-quality care. Licensed RN in California with at least 3 years of related experience is required; a Bachelor's degree is required and a Master's degree is preferred.
