Job Description: Epic Healthy Planet Certification is a MUST HAVERequirement: Epic Healthy Planet Certification (required) Seeking a senior Epic Healthy Planet Analyst with strong population health and value-based care experience.

Required:

Epic Healthy Planet Certification
Healthy Planet build and optimization experience
Registry build and maintenance
Reporting Workbench and dashboard development
Population health workflow analysis
Quality reporting experience (HEDIS, eCQMs)
Care management and care gap management experience
Highly Preferred:
Compass Rose experience
Risk stratification and predictive analytics
Patient attribution frameworks
External data integration (claims, SDOH, HIE feeds)
Medicare Stars and value-based care programs
Clarity/Caboodle experience
Care management tools, care plans, assessments, and outreach workflows

Responsibilities:

Configure and support Healthy Planet registries, risk models, care gap logic, and attribution workflows
Develop Reporting Workbench reports, dashboards, and analytics
Support HEDIS, Medicare Stars, and other quality programs
Build and optimize care management workflows and tools
Validate data between Epic, Clarity, Caboodle, and downstream reporting platforms
Troubleshoot population health reporting and data integrity issues
Partner with clinical, quality, and population health leadership teams
Support upgrades, testing, optimization, and end-user support
