Key Responsibilities:
- Conduct telehealth follow-up visits for geriatric psychiatric medication management.
- Monitor treatment response and adjust medications with attention to polypharmacy, fall risk, and medical comorbidities.
- Perform cognitive screenings and assessments using tools such as MoCA, MMSE, or similar instruments.
- Develop initial care coordination and behavioral support plans.
- Collaborate with nursing staff providing ongoing monitoring and patient support.
- Communicate with the supervising psychiatrist as needed.
- Maintain timely and accurate documentation in the EMR.Required
- Current Florida license and certification as a Physician Assistant or Psychiatric Mental Health Nurse Practitioner.
- Experience providing care via telehealth platforms.
- Strong clinical judgment related to medication safety in older adults.
- Ability to work collaboratively in a team-based care model.
Preferred:
Experience in geriatric psychiatry, long-term care, or dementia care.
Experience with cognitive assessment tools and behavioral management strategies.
Experience working with older adults or in geriatric mental health.
Bilingual in English and Spanish.
Established relationships within the local community.
