As a Licensed Practical/Vocational Nurse with Affinity Hospice Care, you will play a meaningful role in helping patients heal, stay safe, and thrive in the comfort of their own environment. Please note that this role is per diem with a rate of $50 - $60 per visit.
Responsibilities: Provide direct patient care in accordance with the state Nurse Practice Act. Implement and follow individualized care plans established by the RN.Educate patients and their families on self-care techniques and diagnosis-related information. Document care accurately and in a timely manner. Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor. Use supplies and equipment safely, adhering to aseptic technique. Perform other duties as required for patient services.
Requirements: Graduate of an accredited practical or vocational nursing program. Active LPN or LVN license in California.3+ years of nursing experience; home health or med/surg background preferred. Current CPR certification with AHA.Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes. Strong communication skills and a collaborative, patient-first mindset.
