LPN Home HealthThe LPN position provides six patient visits per day. The role requires working within teams of RNs, PTs, OTs, and HHAs. The LPN will deliver care to patients as delegated by the RN Case Manager and/or Clinical Team Manager. This position works under the direction of the Clinical Team Manager. The LPN is responsible for performing skilled nursing functions in accordance with the State Nurse Practice Act and acceptable standards of practice. Responsibilities must align with federal, state, and local laws, along with the guidelines of the professional license. The position requires the completion of one weekend shift per month. Home care experience is a requirement for this role.
