About the Opportunity: We are seeking a Physical Therapist Assistant to join asupportive skilled nursing facility (SNF) rehabilitation team in Petaluma, CA.Working under the supervision of a licensed Physical Therapist, you will carryout individualized treatment plans and help patients progress toward meaningfulfunctional goals.

Responsibilities: Implement treatment plans established by thesupervising Physical Therapist. Provide therapeutic exercise, gait, balance, mobility, and functional training. Monitor patient response and communicateprogress to the Physical Therapist. Educate patients and caregivers within theestablished plan of care. Complete accurate and timely documentation. Collaborate with the rehabilitation andinterdisciplinary care team.
