Chart closure within 72 hours Schedule = 18 PPD, minimum is 18 PPDMonday-Friday 8-5Mix of peds/adults A decision is typically made within 7-14 daysAPC's with 3 years of experience in similar setting (see other AR) BE will be considered if truly BE (5 years or less since residency) Active DEA needed before start Credentialing takes 30-90 days The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years)HCO & Vendor agree to a 30 day notice for ending all assignments.
