Responsibilities:
Conduct comprehensive PT evaluations for residents in a geriatric long-term care setting and establish individualized treatment plans.
Implement therapeutic interventions to improve mobility, balance, gait, strength, and activities of daily living.
Deliver direct patient therapy sessions, track progress, and adjust plans of care as needed.
Document assessment findings, treatment sessions, and outcomes in the facility's electronic health record per policy.
Collaborate with physicians, nursing staff, and therapy colleagues to coordinate care and support discharge planning and transitions.
Ensure adherence to safety, regulatory, and documentation standards while maintaining ethical and professional practice.
Requirements:
- Active Physical Therapist license in good standing; New Hampshire licensure or eligibility for NH licensure
- Graduation from an accredited physical therapy program (CAPTE) and eligibility for licensure
- Ability to work in a long-term care/geriatric rehabilitation environment and engage with residents respectfully
- Strong documentation skills and familiarity with standard PT assessments and modalities
- Availability to work the 35-hour-per-week schedule and participate in a travel assignment
