RN Home Care – Mercy Mercy is seeking a compassionate RN for Home Care to deliver high-quality, patient-centered nursing in the comfort of patients’ homes. Responsibilities include comprehensive assessments, developing and updating care plans, medication administration, wound and chronic disease management, and patient/family education. The RN coordinates with physicians, therapists, and social workers to ensure safe transitions and continuity of care. Provide skilled nursing care in patients’ homes according to physician orders Perform comprehensive patient assessments and document findings accurately Develop, implement, and regularly update individualized care plans Administer medications, treatments, and wound care safely Monitor chronic conditions and respond to changes in patient status Educate patients and families on disease management and home care techniques Coordinate care with physicians, therapists, social workers, and other team members Ensure compliance with home health regulations, policies, and safety standards Use electronic health records for timely and accurate documentation Participate in case conferences, quality improvement, and ongoing training Home health nursing Patient assessment Care planning Wound care management IV therapy and medication administration Chronic disease management (e.g., CHF, COPD, diabetes) Electronic Health Records (EHR) documentation Patient and family education Care coordination and case management Infection control and safety protocols Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in state of practice Associate or Bachelor of Science in Nursing (ASN/BSN) Basic Life Support (BLS) certification Preferred: Advanced Cardiac Life Support (ACLS)
