Social Work Lead: Clinician & IDT Leader

PACE Southeast Michigan in Southfield is seeking a Social Work Lead (LMSW) to assess, coordinate, and deliver social services for participants and families. The LMSW will participate in the IDT and lead daily clinical management and supervision of social workers at the center. The role includes developing treatment plans, conducting assessments, advocating for care, ensuring documentation in the EHR, and guiding care planning while maintaining regulatory compliance and quality improvement.
