To support a growing behavioral health practice, the full-time Florida Licensed Case Manager will assess adult and adolescent clients, coordinate care, and advocate for patient needs while working onsite in Longwood, Florida, with a potential hybrid schedule.
Key responsibilities: Conduct initial and ongoing assessments of clients, including medical history and treatment needs Obtain referrals from behavioral health providers to evaluate client requirements Facilitate family meetings and provide outreach and education at community events
Required qualifications: Bachelor's degree in psychology, social work, or a healthcare-related field One year of experience in case management or related patient care support role Valid Florida driver's license with a clean driving record Bilingual in English and Spanish preferred Ability to maintain compliance with HIPAA regulations and Orlando Health policies
