Responsibilities:
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for residents within the facility
- Implement evidence-based therapy interventions to improve mobility, strength, balance, gait, and functional independence
- Reassess patient progress regularly, adjust goals, and document outcomes in accordance with facility policies and regulatory requirements
- Maintain accurate, timely documentation and collaborate with the interdisciplinary care team to coordinate resident care
- Educate residents and families on home exercise programs, safety strategies, and fall prevention
Requirements:
- Ability to provide physical therapy services in a skilled nursing facility setting
- Strong clinical assessment, treatment planning, and documentation skills
- Effective communication and teamwork abilities to coordinate with nurses, physicians, and other therapists
- Willingness to work on-site in Silver City, NM and adhere to the assigned schedule
