RN Care Manager: Field-Based CKD/ESKD Care Navigator

Somatus is seeking an RN Care Manager, VBC to manage high-needs CKD/ESKD patients. You will work with patients, nephrologists, and practices to develop and implement individualized care plans, remove barriers, and coordinate care across homes, clinics, and offices. You will travel within your region, build trusted relationships with provider teams, and ensure seamless care transitions while delivering patient-centered outcomes.
