State Licensed Senior Epic Analyst

Key Responsibilities: Design and maintain Healthy Planet registries, care plans, and care coordination workflows to support population health initiatives Serve as the primary Epic liaison for stakeholders, translating program goals into technical requirements and facilitating workflow design sessions Mentor junior analysts and validate build work, contributing to team documentation and standard operating proceduresRequired QualificationsBachelor's degree in Health Informatics, Information Systems, Nursing, or related field (or equivalent experience) 4+ years of Epic experience, particularly in Healthy Planet and care coordination modules Epic certification(s) in Healthy Planet, Care Everywhere, or related modules required or strongly preferred Experience designing workflows for population health, care management, or value-based care programs Strong understanding of transitions-of-care processes and cross-continuum care delivery
