What You’ll Do:
- Ensure the highest quality of client care
- Develop and update individualized care plans
- Obtain necessary physician orders for home care cases as required by state regulations
- Monitor and report changes in the client’s medical or mental condition to the attending physician and Agency care team
- Supervise and support the nursing home care team
- Conductweekly nursing orientations
- Conductnursing competency assessmentsin the office or in the client’s home, as needed
- Support the overall clinical needs of pediatric clients
- Promote client safety, independence, and quality of life in the home environment
- What You Bring
- CurrentNew Jersey RN licensein good standingBSN preferred
- CurrentBLS certification
- Minimum 2 years of recent pediatric clinical home care experience
- Recent hands-on experience with advanced pediatric clinical skills, including:
- Pediatric ventilator management
- Tracheostomy careG-tube/PEG tube careIV infusions
- Wound care
- Strong clinical assessment and documentation skills
- Excellent communication and time-management skills
- Ability to work independently in a field-based environment
- Experience supervising and supporting clinical staff
- Strong attention to detail and aclient-first mindset
- Required Health & Immunization Documentation
Candidates must be able to provide: Physical examination completed within the past yearPPD or QuantiFERON-TB test completed within the past year, or a chest X-ray completed within the past five yearsMMR immunization within the past 10 years Affirmed Home Care is an Equal Opportunity Employer.
