TCM Extender - Call Center

What You'll Do: Make outbound and receive inbound calls with members and healthcare partners. Build positive relationships through compassionate, professional communication. Assist Tailored Care Managers by completing outreach, scheduling appointments, coordinating services, and documenting member interactions. Help connect members with healthcare providers, community resources, and essential support services. Maintain accurate documentation while meeting quality and productivity goals.
