Home Health Coordinator- Registered Nurse

Home Health Coordinator- RNSentara Home Care is seeking to hire a qualified individual to join our team as a Home Health Coordinator- RN.Position Status: Full Time - Day Shift Position Location: Suffolk, VAMileage reimbursement will be paid for all miles driven between patient visits. We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The RN Home Health Coordinator coordinates and supervises all nursing care for participants according to program standards. Includes interaction with other facility staff. Actively participates in interdisciplinary team meetings. Conducts initial and periodic assessments. Administer nursing care to ill, injured, convalescent or disabled participants. Advise participants on health maintenance and disease prevention. Administers medication if needed. Education Associate's Degree Nursing (Required) Certification/LicensureRN (Required) - Active in VABLS (Required within 90 days) Experience1-year minimum nursing experience (required) Home Health experience (highly preferred)
