Role Description: This full-time, on-site Nurse Practitioner (Primary Care Provider) role is based in a nursing facility in Harlem, New York, NY. The Nurse Practitioner will provide comprehensive primary care, including conducting routine and acute patient assessments, diagnosing conditions, ordering and interpreting diagnostic tests, and developing evidence-based care plans. Daily responsibilities include managing chronic diseases, prescribing and monitoring medications, performing follow-up visits, and coordinating care with physicians, facility staff, and family members. The role also involves documenting all encounters in accordance with regulatory and facility standards, educating patients and caregivers on preventive care and treatment options, and participating in quality improvement initiatives to enhance patient outcomes.
Qualifications: Active Nurse Practitioner license in New York State and board certification as an NP (e.g., Adult, Family, or Gerontology).Clinical expertise in primary care, including assessment, diagnosis, treatment planning, and chronic disease management in nursing facility or long-term care settings. Strong patient communication and education skills, with the ability to build trust and explain care plans clearly to patients, families, and facility staff. Proficiency in electronic health records (EHR), documentation, and adherence to regulatory and compliance standards. Ability to collaborate effectively in an interdisciplinary team, including physicians, nurses, therapists, and social workers. Demonstrated commitment to preventive care, proactive health management, and person-centered care in diverse communities. Excellent organizational and time-management skills, with the ability to prioritize and manage a steady caseload on-site. Master’s degree in Nursing (MSN) or higher; experience in geriatrics, long-term care, or community health is preferred.
