Essential Functions: Review medications for admission referrals and readmissions. Provide cost of medications to Veterans Land Board (VLB) and obtain approval from VLB to admit if cost of medications greater than $25/day prior to resident being admitted. This is for both services connected veterans and Medicare A residents. Monitor medication ordering to ensure appropriate pharmacy is contacted for medications and medications are ordered in a timely fashion. Monitor new medication orders for current residents to ensure that medication is listed on formulary. If non-formulary obtain authorization for use if formulary of over-the-counter medications and adjust as needed. Maintain monthly list of medication that has been ordered from VLB contract pharmacy
