Responsibilities:
- Provide skilled nursing care to home health and hospice patients
- Monitor patient conditions and report changes to the RN/Clinical Supervisor
- Administer medications and treatments as ordered
- Perform wound care, catheter care, and other skilled nursing interventions
- Educate patients and families on care plans, disease processes, and comfort measures
- Accurately document all patient care and services
- Collaborate with interdisciplinary team members to ensure continuity of care
- Support patients and families with compassion, dignity, and respect
