Music Therapist Hospice

Responsibilities: Develops, coordinates and provides an organized music therapy program. Provides music therapy in alignment with the needs of the patient and physical limitations. Develops a plan of care. Completes all documentation as required by the organization in compliance with legal, governmental and accreditation requirements. Participates in quality assessment and performance improvement activities. Performs other job-related tasks as requested.

Qualifications: Required-Certified music therapist from an established music therapy institute. Licensed driver with reliable automobile which is insured in accordance with state law.
