Registered Nurse

Location: Longmont, CO

Schedule: Monday–Friday | Approximately 25 visits/week

Mileage Reimbursement Provided: Opportunity Overview We are seeking a compassionate and organized Registered Nurse (RN) to provide high-quality home health care and support patients in achieving better health outcomes. This field-based role offers the opportunity to build meaningful patient relationships while maintaining a balanced Monday–Friday schedule.

Key Responsibilities: Manage and coordinate care for assigned home health patients. Educate patients and families on disease management and care plans. Perform skilled nursing assessments, treatments, and follow-ups. Monitor patient progress and provide patient-centered care. Complete accurate clinical documentation and assessments. Collaborate with healthcare teams to support continuity of care.

Required Qualifications: Graduate of an accredited nursing program. Active Colorado RN License in good standing. Minimum 1 year of RN experience required. Home Health experience and OASIS knowledge are a plus. Strong communication and organizational skills. Valid driver’s license and reliable transportation for community-based visits.
